Abstract
This paper is published as a discussion paper. The editors think that some points contained in the paper raise inter- esting questions and issues. The paper is interesting and worthy of publication as the start of a discussion about the contents and conclusions. We therefore actively seek comment, discussion and debate about this paper. Responses will be published in the next issue of InterAction.
